First Visit

We reserve 90 minutes for your initial comprehensive dental examination.  Your dental cleaning will be a separate appointment.  Please bring your completed registration forms, list of medications you are presently taking, and insurance card with you so that your appointment may begin on time.  Otherwise, please arrive 15 minutes prior to your scheduled appointment time to fill out the patient registration materials. If you have removeable dental appliances (retainers, nightguards, dentures) please bring them with you.  (please note: A parent or guardian must accompany all patients under 18 at their initial visit).
 
Click here to download our new patient registration forms and bring them to your first office visit.  If you are unable to do so, please arrive 15 minutes early to allow time for you to fill out your forms at our office.
 
 
Your FIRST VISIT in our office is an important one.  We take time to do a thorough examination and find out what is really important to you, so that our doctors can make the best treatment recommendations for you.  Your initial compehensive exam will include the following:
  •  Discuss your dental concerns
  •  Review your dental history and medical history
  •  TMJ - temporomandibular joint evaluation
  •  Oral Cancer screening (ask us about VIZILITE early detection testing).
  •  Check for cavities (with Laser cavity detection)
  •  Evaluate the condition of existing dental work
  •  Orthodontic assessment: evaluate your bite
  •  Take any necessary X-rays and photographs
  •  Periodontal examination: to see how healthy your gums are and recommend the  appropriate cleaning treatment.
We will always give you an estimate for dental treatment before treatment is rendered.
 
Please alert the office if you have a medical condition that may be of concern prior to your dental visit (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e. antibiotics, for pre-med.)

X-Rays

If your previous dentist has taken recent x-rays (within 6-months), you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
